I always pictured Sethra as appearing rather older and sort of hard-edged... actually quite like what Anne Rice now looks like.  But, since she is, as you say, older than dirt, a premier sorcerer, and undead to boot, there is no reason she should not look like this. But I think I've always thought of Sethra as very much like a marble statue with an Allison Janney skull... and her amused expression.  This Sethra's much softer than that, but still nice.
